Acumen Halts Its Only Drug After 542-Patient Alzheimer's Trial Fails

Acumen Pharmaceuticals will stop developing sabirnetug, its only drug in clinical testing, after a 542-person phase 2 trial in early Alzheimer's disease failed. The company reported the result on October 30, 2026, and its shares fell sharply in early trading.
What happened
The ALTITUDE-AD trial tested sabirnetug, an antibody aimed at soluble amyloid beta oligomers. Acumen says these are toxic forms of amyloid. The drug had FDA Fast Track status for early Alzheimer's disease.
The study enrolled people with mild cognitive impairment or mild dementia due to Alzheimer's at sites in the US, Canada, the EU and the UK. Participants received an infusion every four weeks.
- Design: two doses, 35 mg/kg and 50 mg/kg, against placebo, with about 180 people per arm.
- Main goal: change from baseline on the iADRS scale of thinking and daily function at 18 months.
- Result: neither dose showed a meaningful difference from placebo. Key secondary measures, including CDR-SB, were not statistically significant.
- Safety: Acumen called the drug generally well tolerated. Brain swelling seen on scans, known as ARIA-E, occurred in about 10% of treated participants.
Both doses did lower amyloid plaque on PET scans by a statistically significant amount at weeks 36 and 76. That did not translate into a clinical benefit. Acumen expects more biomarker and imaging data in the coming weeks, but said it does not expect them to change the efficacy conclusions.
Chief executive Daniel O'Connell said the company was disappointed and thanked patients, caregivers, trial sites and staff.
Why it matters
BioSpace reported that sabirnetug was Acumen's only pipeline program, and that the stock fell 80% to 44 cents per share in pre-market trading after the news. The company is also ending the open-label extension of the trial.
The readout came earlier than planned. In its third quarter report on October 26, Acumen said it expected topline results in November 2026.
In this trial, lowering amyloid plaque was not enough to slow decline over 18 months.
What it means for people in the industry
Acumen said it will take steps to conserve cash and look at strategic options, including its Enhanced Brain Delivery program. It did not give details on staff changes.
The company had $78.6 million in cash, cash equivalents and marketable securities as of September 30, 2026. Before the readout, it said that money would fund its activities into early 2027. Its third quarter net loss was $25.4 million.
Things to watch include any workforce update, the outcome of the strategic review, and the remaining biomarker data.
